Our public website is for general information, scheduling requests, and business inquiries. Clinical information is handled separately.
General website forms
Our public website forms are for general, non-clinical inquiries. Please do not submit protected health information (PHI), diagnoses, medical history, evaluation reports, insurance details, photographs of records, or other sensitive clinical information through these forms.
For school and district inquiries, do not submit student names, IDs, educational records, medical information, or other protected student data through the public website.
Clinical intake & secure teletherapy
Family clinical information is collected through Carepatron rather than through the public website. Carepatron is also the platform Poco a Poco uses for secure teletherapy when telepractice is clinically appropriate and available.
Client access to health records
How to request your records
Current clients, parents, legal guardians, and other authorized personal representatives may request access to health records through the secure Carepatron client portal provided by your SLP.
- Sign in to the Carepatron client portal using the secure access information provided by Poco a Poco Therapy.
- Send your records request through the secure portal or follow the records-request instructions provided there.
- If you do not have portal access, email pocoapocotherapypllc@gmail.com and ask for secure portal access for a records request. Do not include diagnoses, records, or other PHI in ordinary email.
- Poco a Poco Therapy will process requests for access, copies, amendments, or other record-related rights in accordance with applicable federal and Texas law.
Texas law provides certain rights regarding privacy of and access to health care records (Texas Health & Safety Code, Chapter 181, Medical Records Privacy). Contact the Texas Office of the Attorney General, Consumer Protection Division to file a consumer complaint alleging a violation of your privacy rights regarding protected health information.
Report suspected violations of statutes or rules enforced by the Texas Department of Licensing and Regulation by contacting the Department or calling (800) 803-9202.

How website submissions are used
Information submitted through general website forms is used to respond to your inquiry, discuss availability, or follow up about a requested service, school contract, or career inquiry. Only information reasonably needed for that purpose should be submitted.
