Notice of Privacy Practices.

Effective date: September 3, 2026

Applies to: Poco a Poco Therapy PLLC and its workforce when handling protected health information on behalf of the practice.

Your rights

You have rights regarding your health information. Depending on the request and applicable law, you may:

• Ask to inspect or receive an electronic or paper copy of your medical record and other health information we maintain about you.
• Ask us to correct information you believe is incorrect or incomplete.
• Ask us to contact you in a specific way or at a specific location.
• Ask us to limit certain uses or disclosures of your information. We are not required to agree to every request, except in circumstances required by law.
• Ask for an accounting of certain disclosures of your health information.
• Ask for a paper or electronic copy of this notice at any time.
• Choose an authorized personal representative to exercise rights on your behalf, when permitted by law.
• File a privacy complaint without retaliation.

Records requests: Use the secure Carepatron client portal provided by your SLP. If you do not have portal access, email pocoapocotherapypllc@gmail.com and ask for secure portal access for a records request. Do not send PHI by ordinary email.

Your choices

For some types of sharing, you may tell us your preferences—for example, sharing information with family members or others involved in your care or payment for care. When written authorization is required, we will obtain it before using or disclosing your information for that purpose. You may revoke an authorization in writing as allowed by law.

Poco a Poco Therapy does not sell protected health information. We do not use or disclose PHI for marketing in a manner that requires authorization without first obtaining the required authorization.

How we may use and share health information

Treatment

We may use and share health information to provide, coordinate, or manage your care and to communicate with other professionals involved in treatment when permitted. For example, with appropriate permission or as otherwise permitted by law, an SLP may communicate relevant information to another treating provider.

Payment

We may use and disclose information to bill for services, process payments, verify benefits, support claims, provide superbills when applicable, or communicate with health plans and other payers as permitted.

Health care operations

We may use and share information to operate the practice, improve quality, coordinate scheduling, conduct compliance activities, train appropriate workforce members, and manage clinical and administrative services.

Other uses and disclosures permitted or required by law

We may use or disclose information when permitted or required for public health and safety activities, reporting suspected abuse or neglect, health oversight, workers’ compensation, certain law-enforcement or government requests, legal proceedings, research when legal requirements are met, and other situations authorized or required by law. We may disclose information to the U.S. Department of Health and Human Services when necessary to demonstrate compliance with federal privacy law.

If Poco a Poco Therapy receives substance use disorder patient records protected by 42 CFR Part 2, those records will not be used or disclosed in civil, criminal, administrative, or legislative investigations or proceedings against the patient unless permitted by applicable Part 2 requirements, such as with valid written consent or an appropriate court order and subpoena.

Our responsibilities

• We are required by law to maintain the privacy and security of protected health information.
• We will notify affected individuals as required if a breach occurs that may have compromised the privacy or security of protected information.
• We must follow the duties and privacy practices described in the notice currently in effect.
• We will not use or share information other than as described in this notice unless the individual authorizes us in writing or another law permits or requires the use or disclosure.
• We comply with applicable federal and Texas privacy requirements, including Texas Health & Safety Code Chapter 181 where applicable.

Complaints

You may submit a privacy complaint to Poco a Poco Therapy using the Privacy Office contact information above. We will not retaliate against you for filing a complaint.

You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights. HHS Office for Civil Rights complaint information.

Changes to this notice

We may change the terms of this notice, and changes may apply to all information we maintain. The current notice will be available on this website and upon request.
